If you need Attendance Verification for your student
please allow 24 hours for the letter or form to be filled out.
You may send you request via email, call or come in to request.
Social Service requests are mailed back as indicated on the form.
Social Service forms are mailed out within 24 hours to the address indicated on the form.

When a student is absent parents should call the Attendance Office leaving the spelling of the student's first and last name along with the date and reason for absence.
If a student has a medical or dental appointment or needs to leave campus for any reason a note must be delivered to the attendance office the morning of the release BEFORE school begins.
The attendance office will give your student a pass to leave at the time indicated on the note. This will eliminate interruptions and distractions to the class.
If this is a last minute appointment you need to come to the attendance office to sign your student for release.
If your student is returning to school from a medical or dental appointment, please have your student bring the doctor's note to the attendance office so we can clear the absences.
Attendance Clearance Policy
| 1) All excusable absences must be cleared by telephone call or a note from the parent/guardian within 72 hrs (3 days) of the student’s return. | |
| 2) Medical Note. No more than 3 consecutive days of absence can be cleared for medical reasons. A Doctor's note must be provided for more than 3 consecutive days. | |
| 3) Off Grounds Passes for Appointments. If the parent/guardian is aware of the appointment in advance, PLEASE send a note with the student which indicates the date, time and reason for leaving. Student must bring the note to the attendance office before school starts in the morning. The student will then be issued a note to show to their teacher during the appropriate period. The note will minimize interruptions to the classroom during class time. | |
| 4) Student’s must have a “Pass to Leave School Grounds” from the office to leave campus. Student’s leaving campus without a pass cannot be cleared. Student will receive cuts for periods unexcused. |

School Attendance Review Board (SARB)
Hayward Unified has CWA (Child Welfare and Attendance) in place to assist with the SARB process which helps develop ways of coordinating school, community and home efforts to deal with student attendance and behavior problems. The Education Code for the State of California deals with truancy in the following manor:
48260 Any pupil subject to compulsory full-time education or to compulsory continuation education who is absent from school without valid excuse three full days in one school year or tardy or absent for more than any 30-minute period during the school year or any combination thereof, is a truant and shall be reported to the attendance supervisor or to the superintendent of the school district.
48290.5 The parent or guardian is obligated to compel the attendance of the pupil at school. Parents or guardians who fail to meet this obligation may be guilty of an infraction and is subject to prosecution. Parents or guardians have the right to meet with appropriate school personnel to discuss solutions to the pupil’s truancy.
48261 Any pupil who has once been reported as a truant and who is again absent from school without valid excuse one or more days, or tardy for more than any 30-minute period on one or more days, shall again be reported as a truant to the attendance supervisor or the superintendent of the school district.

48262 Any pupil is deemed an habitual truant who has been reported as a truant more than three times per school year, provided that no pupil shall be deemed an habitual truant unless an appropriate district officer or employee has made a conscientious effort to hold at least one conference with a parent or guardian of the pupil and the pupil himself, after the filling of either of the reports required by Section 48260 or Section 48261.
Students found in violation of violating the Californian Ed. Code attendance and /or tardy regulations may be requested to appear before the district’s School Attendance Review Board, SARB.
SARB hearings are scheduled to take place at the Hayward Police Department when all attempts have failed to remedy the attendance / tardy problem.
Student in grades 11 and 12, who fail to comply may have their work permit revoked which can result in the district petitioning the court to revoke or delay the issuance of a California Driver’s License. (Vehicle Code 13202.7
